How we protect our patient’s information.

What do I need to know?

First Source for Women is committed to respecting the privacy of our Patients. We want to ensure our patients that information will not be shared with any third party without your consent. As the patient, it is your business to know what information we collect and how we use it.

What information do we collect and how do we use that information?

We collect patient names and contact information. The information could then be used for the following: treatment options, contacting you about appointments or results, notification of upcoming events, as well as additional notices throughout the year if the patient requested updates.

How can donors contact us to review their personal information and request corrections?

If you have questions concerning your personal information, please contact First Source for Women at our main number (256-352-5683). Our staff will be glad to help.

How can patient’s inform us that they do not want their information shared outside of First Source for Women?

We do not share our patient information outside of First Source for Women without prior consent.

What security measures do we have in place to protect patient information?

All patient information is maintained in house in a lockable file with limited access and stored securely online in our patient portal software.

How we protect our donor’s information.

What do I need to know?

Get all the facts. It is important to remember that abortion is a medical procedure. Once you have a surgical abortion, it cannot be undone. You owe it to yourself to make an informed decision, and to do that, you need all the facts. We will help you answer questions such as:

What information do we collect and how do we use that information?

We collect donors’ names and contact information on the pledge forms. The information could then be used for the following: a thank you letter, a tax letter, solicitation during the next campaign, notification of upcoming events, as well as additional notices throughout the year if the donor asked their gift to be designated and did not request to remain anonymous.

How can donors contact us to review their personal information and request corrections?

If you have questions concerning your personal information, please contact First Source for Women at our main number (256-352-5683). Our staff will be glad to help.

How can donors inform us that they do not want their information shared outside of First Source for Women?

We do not share our donor information outside of First Source for Women, aside from the Annual First Source for Women Banquet recognition, in which we thank our underwriters and list their names. For those that wish to not be listed in this report, there is an area on the pledge form for the donor to indicate that they would like to remain anonymous.

What security measures do we have in place to protect donor information?

All donor information is maintained in house (including credit card information) in a lockable file with limited access for 5 years. Donor information is only used for tracking donor pledges and payments. No specific donor information is shared outside of First Source for Women. Information is shredded on site after 5 years.
